and youth homes, homes for the elderly, multi-service
centres for the elderly, community centres, rehabilitation
centres, homes for the disabled, as well as sheltered
workshops for the disabled, etc. |

Social Service Facilities Management and Licensing
Division, and submits: |

of academic qualification (if the applicant
is an individual); |

Certificate
of criminal record (if the applicant is

Document
issued at Identification Department (if
the applicant is an association); |
 |
Photocopy
of the organizational statute published
in “Macao Special Administrative Region
Government Gazette” (if the applicant
is a legal entity); |
 |
Register
or other relevant document at Finance Services
Bureau in previous year (if required by
law); |
 |
List
of technical and auxiliary staff estimated
for social service facility; |
 |
Fees
and charges table appropriate for the operation
of first year; |

Other
technical documents and information required

Decision
will be made within 45 days counting from the
date of receiving the application form and all

according to legislation, to present sufficient
documents and information. Counting will resume
upon receiving the additional information. |

| Alteration
of Licence Holder |
| |
The original licence
holder has to fill in the application form, and
submits: |

of transfer of ownership of social service
facility; |
 |
Certificate
of academic qualification (if new licence
holder is an individual); |

Certificate

of Content of Licence |

The relevant
licence holder has to make a written application
to Social Service Facilities Management and Licensing

of the licence (written by the applicant); |

Relevant
declarations and technical documents and

| |
The licence
is valid for one year. Social Welfare Bureau
will notify licence holder or his/her representative
of non-renewal decision 30 days before expiry
date of the licence. Otherwise, the licence will
be renewed automatically. |

and Documents Required for Replacement |
| |
Licence
holder has to fill in and submit the application
form to Social Service Facilities Management and
Licensing Division. If the licence is damaged,
the original of licence has to be returned. |
